ISQSC 2012 REGISTRATION PAYMENT BY PAYPAL

To use the PayPal option you must

1. Complete your registration form and note total amount due.
2. Use PayPal Donate button to make your payment and note payment transaction number.
3. Add transaction number to registration form.
4. Mail form to ISQSC Registration, PO Box 2662, Fargo ND 58108.
5. Registration forms without a transaction number may not be processed in a timely manner.
